APPLICATION INFORMATION

Application for Admission to MSU            **YOU MUST APPLY TO MSU & TO THE PROFESSIONAL PGM**

Complete the Midwestern State University  Application for Admission on-line. It must be received by the application deadline described in the application information on-line.. If you have less than 18 earned hours, you must meet the beginning freshman criteria by furnishing the Admissions Office with your high school transcript and a copy of your ACT/SAT scores. Beginning Freshmen over the age of 21 may have the ACT/SAT requirement waived if they meet all the unit and class rank requirements.

Official Transcripts

Official transcripts are immediately required from each and every institution attended and must be mailed from the issuing institution (not hand-carried) directly to our Admissions Office. If you take courses at a local institution while enrolled in the BSRS program, you must update your records with our Admissions Office by ordering an official transcript after each semester of attendance at the local institution.

Official Withdrawal

If you find that you are unable to continue, it is imperative that you officially withdraw immediately as your grade determination will be based upon the date the official withdrawal form is processed. Failure to officially withdraw will result in an automatic grade of "F" in the course. Instructions concerning withdrawal of Radiological Science Courses will be included with your on-line course materials.

Individuals applying to the Radiologist Assistant Program should complete the RA Program Application linked on the RA Program website.  Admission to the RA Program is limited and competitive.
